Exclusions and Limitations
- “Deep cleanings” are not included in the adult plan. If required, they are assessed separately and qualify for the perio plan.
- A prophylaxis (routine preventive cleaning) cannot be performed on a member with periodontal disease, as its purpose is to prevent periodontal disease.
- If you are diagnosed with periodontal disease, you will need either a deep cleaning or periodontal maintenance.
- The 25% discount on scaling and root planing will apply to any required deep cleaning.
- Treatment that cannot be performed due to the member’s general health or physical limitations, as determined by the member’s physician or Buchanan Dentistry, is excluded.
- Experimental dental procedures are not covered.
- Dispensing of drugs is not included.
- Treatment covered by Workers’ Compensation, employer liability laws, government agencies, or other insurance carried by the member is excluded.
- General anesthesia, oral conscious sedation, and nitrous oxide are not included.
- Any dental care requiring referral to a specialist is excluded.
- Buchanan Dentistry reserves the right to refuse treatment to members who do not follow a prescribed course of treatment.
Dental treatment started before the membership effective date is not covered, including but not limited to:
- Teeth already prepared for crowns
- Root canals already in progress
- Orthodontic treatment already underway
- Exclusions and treatment fees are subject to change without notice.
- If a member obtains and uses any third-party insurance plan during the contract period, this membership cannot be used and remains non-refundable.
- Membership plans are only valid at Buchanan Dentistry.
- Membership benefits are non-refundable and only valid during an active membership and regular business hours at Buchanan Dentistry.

How do I use my membership plan for my treatment?
When scheduling an appointment, please let us know that you're a member and what plan you have. When you come in for your visit, we’ll apply the necessary discounts.

Can I transfer membership plan membership to another dentist?
No, our membership plan options are only valid at Buchanan Dentistry.

Can I combine my membership plan discounts with dental insurance?
Our membership plan cannot be combined with dental insurance or other discounts.

What are my responsibilities as a member?
Entire membership fee must be paid upfront and is valid for 12 months.
Membership fee is non-cancelable and non-refundable.
To receive discounts on treatment, treatment must be paid in full on the day of service; no exceptions.
Failure to show up for or cancel a scheduled dental cleaning appointment without the required 24-hour notice will count as one of your dental cleaning occurrences.
Missed or late-canceled dental cleaning occurrences cannot be made up.
